Would you like to have a say about the services provided at Tamar Valley Health?

We would like to contact patients periodically to learn about their opinions of the Practice and how well they feel we are providing our services, in order that we can identify areas where patients think we can make improvements. This information will be gathered using on-line surveys and at no time will any personal questions be asked. Any changes that are made as a result of these surveys will be purely patient-focused.

If you are interested in participating in our online surveys, please complete the form at the bottom of this page. We will then add your details to a contact list, which will be kept safely and securely. Your details will only be used for this purpose and will not be shared with anyone else. We will only contact you occasionally, to advise that we have posted an on-line survey and you can, of course, choose to ignore such messages.

You can stop contributing to the Patient Participation Group at any time by letting us know that you no longer wish to receive e-mail reminders.
